Transcripts & Student Records

Freshmen, Sophomores, Juniors, Prior SMHS Students

To request a copy of your transcript, please fill out and submit this Google Form.

Current SMHS Seniors

Please follow the instructions below to request a transcript:

  • Go to parchment.com/register, enter your registration code and click "Sign Up"
  • Step 1: Complete Your Profile
    • Required Information: full legal name, date of birth, graduation year, grade level, email address, create a secure password
    • You'll be asked if you want to waive your FERPA rights for future documents like letters of recommendation
      • Waive - You choose not to view certain documents (often recommended for college applications)
      • Do Not Waive - You keep the right to view those documents in the future
  • Step 2: Complete Final Information
    • Enter the following: contact information (phone, address), academic interests (optional), GPA (required), SAT/ACT Information (optional). Click "Continue" to move forward
  • Step 3: Log Out and Log Back In to Refresh Account
  • Step 4: Request Transcript
    • Select "Order a Transcript"
    • Search for the college you want to send your transcript
    • Complete the required information when prompted (first legal name, last legal name, month of birth, day of birth, year of birth) Are you currently enrolled? Yes, Currently Attending. Add your first year of attendance and expected graduation year then verify your name. Click "Continue" (don't check the checkbox)
    • When do you ant this sent? Hold For Grades (The school will send the transcript once the final grade are posted)
    • Sign your name electronically, click "Continue"
    • Click "Complete Order"

To Request an Unofficial Transcript

You also have the option to request an unofficial copy of your transcript. To do that, you will need to select the "I'm sending the order to mysejf" option and follow the instructions shown above in Step 4.
